This policy explains what Essential Coverage Inc collects through this website, why, and what you can do about it. It covers this website. Information you give us later, during an application or enrollment, is handled under the separate privacy notice we deliver at that point, as New York insurance regulation requires.
What we collect here
Only what you type into the consultation form:
- Your first and last name
- Your email address
- Your street address, city, state, and ZIP code
- The Medicare topic you select
- Whether you are already enrolled in Medicare, if you choose to say
- Any date or time preference, and anything you write in the notes field
What we do not collect here
We do not ask for your telephone number, and this website has no field for one. We do not ask for your Medicare Beneficiary Identifier, Social Security number, date of birth, driver's license, bank or card details, prescriptions, diagnoses, or any other health information. Please do not send those through this form or the chat. If any of it reaches us anyway, we delete it and ask you to resend it securely.
Why we need each piece
- Name and email — to reply, confirm a time, send you the Scope of Appointment form, and send your written summary afterward.
- Address and ZIP code — Medicare Advantage and Part D plans are sold county by county, and across New York City the available plans and their provider networks differ by borough. Your ZIP code determines which plans you can actually join, so we cannot give you a useful answer without it.
- Topic and enrollment status — so the appointment starts in the right place and the Scope of Appointment covers the right product types.
We do not use your information to build advertising profiles, and no automated system makes decisions about you.
We do not sell leads
We do not sell, rent, or trade your personal information. We do not share it for cross-context behavioral advertising, and we do not pass it to lead buyers, call centers, or other agencies. Under federal Medicare rules, personal beneficiary data collected for marketing or enrollment may be shared with another third-party marketing organization only with your prior express written consent, given through a clear disclosure naming each recipient. We do not ask for that consent on this website and we do not share your data this way.
Who else sees it
- Clark Guo and authorized staff of Essential Coverage Inc.
- Service providers that host this website, run the chat window (LeadConnector), deliver our email, and store our records. They may use your information only to provide those services to us and are bound to protect it.
- An insurance carrier, but only if you ask us to pursue a specific quote or enrollment, and only with your knowledge.
- The Centers for Medicare & Medicaid Services, a carrier conducting the oversight of its agents that federal rules require, the New York State Department of Financial Services, auditors, or a court, where the law requires disclosure.
Recorded conversations
Federal Medicare rules require agents to record marketing, sales, and enrollment calls in full, including the audio of video calls. If we speak with you about Medicare plans by phone or video, the call is recorded and stored securely with restricted access. Marketing and sales recordings are kept for at least six years, with audio for the first three years and audio or a complete transcript for years four through six. Enrollment records are kept for ten years. These recordings are used only for compliance, oversight, and quality purposes.
Cookies and tracking
This site does not use advertising cookies or third-party tracking pixels. The chat window on each page is provided by LeadConnector, which may store small files in your browser so the chat works. If you use the chat, what you type there, along with any contact details you choose to give, reaches us through LeadConnector. You can use the rest of the site without opening the chat. Our web host records standard technical details such as IP address and browser type in server logs for security and troubleshooting.
How long we keep it
If you become a client, we keep your records for at least six years, and longer where federal Medicare rules require it, including ten years for enrollment records and Scope of Appointment forms. If you do not become a client, we delete consultation requests after twelve months. You can ask us to delete yours sooner and we will, unless a law or Medicare rule requires us to keep a specific record.
How we protect it
This site is served over an encrypted connection. Submissions go to a limited-access mailbox, records are stored with access restricted to people who need them, and devices are secured and kept current. As an insurance producer licensed in New York, this agency is subject to the Department of Financial Services cybersecurity regulation and to New York's data security law, and maintains safeguards accordingly. No system is perfectly secure, which is exactly why we keep sensitive identifiers off this form in the first place.
If something goes wrong
If your personal information is exposed in a security incident, we will notify you and the appropriate New York authorities within the time the law requires, tell you what happened and what was affected, and tell you what to do about it. We will not quietly sit on it.
Your rights
You may ask us to tell you what we hold about you, give you a copy, correct it, delete it, or stop emailing you. Write to contact.health.clarks@gmail.com. We respond within the time the applicable law requires, generally within 45 days, and we will not treat you differently for asking. Under New York insurance regulation you also have the right to see and correct the personal information we hold about you, and to have a disagreement recorded in your file if we decline a correction. Additional rights may apply depending on where you live.
Helping someone else
If you submit this form for a parent or relative, we will ask for their authorization before we discuss their coverage or collect anything further about them. A Medicare beneficiary's own permission is required regardless of who fills in the form.

Free language assistance and auxiliary aids are available. Write to contact.health.clarks@gmail.com and we will arrange an interpreter or an alternate format at no cost to you.
Children
This site is meant for adults. We do not knowingly collect information from anyone under 16.
Changes
If we change how we handle your information, we will update this page and the date at the top, and describe material changes plainly rather than burying them.
